Fixed Stuck OAuth Reconnections on Permanently Rejected Tokens

When an upstream MCP server permanently rejected a stored refresh token, MCPX previously kept the dead token and looped on "Server unreachable" instead of prompting re-authentication.

  • MCPX now detects permanent rejections and clears the stored tokens.
  • Users are routed to fresh OAuth instead of getting stuck.
